Release of Liability and Physical Activity Consent

Participation in any training activity is voluntary. All participants are strongly encouraged to undergo a health evaluation and consult with their personal physician to determine the appropriate fitness level for strenuous activity prior to participation.

Release of All Claims for Personal Injury or Property Damage

By participating in any training activities associated with Vital Health Skills, I acknowledge and accept the risks of personal physical injury, property damage, or other losses, including death. In consideration of being allowed to participate, I agree to the following:

1. 
   

   Assumption of Risks: I assume all risks associated with any personal injuries, property damage, or other losses that may result from my participation in any activities associated with the programs offered by Vital Health Skills.

2. 
   

   Release of Liability: I fully release and discharge Vital Health Skills, its officers, agents, and employees from any claims for personal injuries, property damages, or other losses resulting from my participation in any activities provided by Vital Health Skills.

3. 
   

   Indemnification: I agree to indemnify and hold harmless Vital Health Skills, its officers, agents, and employees from all claims, suits, actions, injuries, damages, and losses arising from or connected to my participation in any activities associated with the programs offered by Vital Health Skills.

License

I hereby grant Vital Health Skills a permanent, royalty-free, non-exclusive license to use my likeness in photographs, videos, or any other medium for any and all publications, whether in existence now or in the future. I will make no monetary or other claim against Vital Health Skills for the use of my image or voice.

Payment Terms of Agreement

By engaging in business with Vital Health Skills, you agree to the following terms:

1. 
   

   No Refunds: Full payment is required at the time of registration to secure your spot in the class. Registration is only confirmed once payment is received. All sales are final.

2. 
   

   Rescheduling Policy: Students are allowed one (1) reschedule of a course with prior approval from the instructor. Contact your instructor to see if rescheduling is an option based on future course availability. A rescheduling fee of $15.00 applies. Reschedule requests must be made at least 24 hours before the course start time. Additional reschedules may incur further fees.

3. 
   

    Minimum Class Size Requirement: All classes are subject to a minimum participant requirement. If the minimum class size is not met, we reserve the right to reschedule the class to another date. You will be notified of the rescheduled date and given the option to either:
   -Attend the rescheduled class, or
   -Pay the private session rate to keep your original registration date.
   -In the event that the minimum class size is not met and rescheduling or a private session is not suitable for you, a full refund will be provided.
   Private Session Option: If you wish to proceed with a private session due to the class being rescheduled or the minimum participant requirement not being met, you will be charged the standard private session rate. Private session rates will be provided to you upon request.

4. 
   

   In the Event of Instructor Cancellation: If a course is canceled by Vital Health Skills (due to instructor illness or any other reason), a rescheduling option will be provided to all registered students. In the event rescheduling is not an option a refund will be provided.

By participating, you acknowledge that you have read, understood, and agree to these terms and conditions.
